Medical Information for a Traveler, Field Definitions
Doctor's Name, Phone should be used for the doctor to reach during business hours. A 24-hour number can be entered under Emergency Contact.
Emergency Medical Note: use this field to indicate current medications and allergies to medicine.
Health note: store general health information here, including mobility limitations, chronic conditions, etc.
Dietary note: store information regarding dietary requests, allergies, etc.
Emergency Contacts: people to contact in an emergency. Up to two records are displayed on included reports.
Emergency Phone: enter phone and type.
Emergency Contact Relationship: select a relationship from the list. If you are adding a new relationship on the fly, you will also want to add the "reciprocal" relationship to the code class under code category. See Relationship Code Class in Code Category Editing, Exceptions
